There is no ID card, no group or member numbers to bring! All your membership information will be kept in your electric record. your effective date is the day you sign up and your renewal date is the same date every year.

Terms & Limitations to the Plan
- This is a dental discount plain is NOT dental insurance. It cannot be combined with any other dental insurance.
- This plan is good only for Innovative Dental Care therefore if you are referred to a specialist, they will NOT honor this discount.
- Should there be dental treatment needed following any type of injury where a lawsuit and/or workman’s comp type insurance are involved, this discount plan cannot be used.
- This is NON-Transferable. Family members cannot be substituted in for another family member.
- This plan is NON-Refundable. No refunds will be given if patient chooses not to use their dental plan.
- Rates are subject to change annually.
- Payment for services are due at time of service. If you choose to extend your payment for treatment by paying through CARE CREDIT the discount will be reduced by 10% due to merchant fees.
- This offer cannot be combined with any other offers.
- For orthodontic treatment, participants must remain a plan participant the entire duration of orthodontic treatment.
- ONLY dental services are covered, products are NOT included in discount plan.
- Aesthetic services (Botox, Juvederm, etc.) are NOT included in discount plan.
